01 / Why we started

Your team didn't get into healthcare to chase faxes, documentation, calls, third-party ancillaries and desk work.

Every day, someone in your office is calling a home health agency about a missing signature, re-faxing an order that came back incomplete, or sitting on hold with a supplier. It has to get done, but it isn't patient care, and it eats up the day. Sebastian Network takes that work off your plate and handles it from start to finish.

Sound familiar?

“I can't find the faxed order home health sent yesterday.”
“Another call from home health about the signature?”
“The DME company called again about that walker order.”
“Pharmacy's on line two about a refill.”

What we take
off your desk.

Seven kinds of work.
One local network.

01 / Incoming paperwork

We check the paperwork before it gets to you.

Orders, plans of care and referrals often show up missing a signature, a diagnosis code or a date. We catch what's missing and go get it, so what lands on your team's desk is ready to use.

02 / Signatures

Forms come to you ready to sign.

No more flipping through a 485 to work out what's missing. Home health, hospice and DME paperwork reaches you complete and in order, so you can sign it and move on.

03 / Outside calls

We take the calls from everyone else.

Home health agencies, hospices, DME suppliers and pharmacies all need something from your office. We take those calls and messages and follow up for you, so your front desk can look after patients.

04 / Follow-ups

Nothing gets forgotten.

We keep track of every open request and stay on it until it's done. No more sticky notes or “I thought you called them.”

05 / Billing support

Get credit for work you already do.

Care coordination often goes undocumented. We keep the notes up to date so eligible work is easier to support and bill.

Offer more care without more paperwork.

We handle the day-to-day admin behind remote monitoring, chronic care management and transitional care for eligible patients.

07 / Status

Know where everything stands.

One place to see what's done, what's still waiting and what needs your attention.

02 / Why it matters

Spend your day with patients,
not paperwork.

Longer visits. Less rushing to the next chart.

Fewer charts to finish at home. Evenings back.

03 / What admin costs a practice

The paperwork
doesn't stop at 5.

These numbers come from published national research. If they sound like your office, you're not alone.

04 / Sebastian

Built for
Sebastian.

Sebastian is a small town with a lot of retirees and not enough doctors. More than a third of residents are 65 or older, and Indian River County is a federal health professional shortage area. Patients here can wait months to be seen. Every hour your office spends on paperwork is an hour a patient could have had.

06 / Questions

Common questions.

What does it cost to join?

Nothing. Practices join Sebastian Network for free. It's a local network, not a software subscription.

No. We work with the EHR you already have, including Epic, athenahealth, eClinicalWorks and ModMed. Your team keeps working the way it does today.

How do you protect patient data?

We're HIPAA compliant and SOC 2 Type II certified. Your records stay in your EHR, and nothing gets moved to get started.

Is AI making clinical decisions?

No. AI only helps with the admin work around care, like paperwork, calls and follow-ups. Every clinical decision stays with you and your clinicians.
